Temporary changes to the Practice – from Monday 6 April 2020 for 12 weeks

As you are aware the NHS is currently dealing with the Coronovirus pandemic and in order to help us to help you, the practice needs to make some temporary changes to continue to provide healthcare and advice when you and your family need it most.

From Monday 6 April, you will NOT be able to walk into this practice any longer.

Should you need to speak to your practice, you will need to call the practice 0121 554 1812

If you have a medical problem you will be assessed by a GP/Practice Nurse over the telephone.

If they decide you need to be examined they will make an appointment for you at Tanhouse Clinic, Hamstead Road, Great Barr.

The NHS is limiting Face-to-Face consultations due to Covid-19.

It is important that we all do our bit to support the NHS locally as it continues to see and treat you, your family and friends.

New Patient Registration

If you live in our practice area and would like to register with us, please complete one of our “New Patient” registration forms available at reception or downloadable below.

The surgery accepts patients living within a mile radius of the surgery. Patients moving more than 1.5 miles from the surgery will not be able to remain with the practice.

On the form, you will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP. We have a team of highly qualified and experienced practitioners who can help you. You will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

New patient registration pack

New patients are required to have a health check on joining. This allows us to record any relevant information while we wait for your records to be transferred from your previous GP.

Please bring in your completed new patient registration forms to the surgery so that we can book you an appointment for your health check. Forms will not be accepted via email/post.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
